Rural Primary Health Services – Care Coordination Program
Care Coordination involves:
- Care Coordinators working with you, your GP, and other health providers to assess your health and care needs.
- Care Coordinators find and arrange services you need to stay health and independent.
Care Coordinators can help you to plan and manage your health and wellbeing, access services, and provide support to help you understand your condition.
We have a no wrong door policy, meaning we will assist you by either referring you in-house to Rural Health Tasmania Programs or to other external services that can assist you.
Chronic Conditions can include: Musculoskeletal, Cancer, Mental Health, Circulatory and Respiratory conditions
Under the Care Coordination program, you will have access to Registered Nurses and Mental Health Clinicians.
This service is supported by funding from Primary Health Tasmania through the Australian Government’s PHN Initiative.

People with living with chronic conditions and their families and/or carers
This service is available to all ages, including young people and over 18 years of age

9am-5pm Monday-Friday
