Mindset – Choices

Choices is an intensive, short-term psychosocial outreach and accommodation service for people exiting an inpatient mental health stay and are homeless or need intensive support to prevent readmission.

Duration: 3 Months Max for psychosocial support, Accommodation for 2 months
