Mental Health Nurse Access Program (MHNAP)
Our Mental Health Nurse Access Program (MHNAP) is here to support adults dealing with serious mental health conditions by providing free assistance and coordination of care. Here’s how we can help:
- Easy Access to Care: Get assessments, referrals, and help connecting with doctors and specialists
- Support for Your Health: Assistance with medication management and coping with mental health conditions
- Advocacy and Support: We’re here to advocate for you and support your health journey
This service is supported by funding from Primary Health Tasmania through the Australian Government’s PHN Initiative.
Who this service is for
Eligibility:
- Adults over 18 with a diagnosed serious mental health condition.
- A GP mental health treatment plan is needed. We can help you get started.
- Location: We serve several areas of North-Western Tasmania including Circular Head, Waratah-Wynyard, West Coast, King Island, Burnie, Central Coast, Devonport, Kentish and Latrobe.
