Corumbene Rural Primary Health
Corumbene Rural Primary Health Program is designed to help people living with chronic health health conditions in the Derwent Valley, Central Highlands or Southern Midlands to:-
- Better manage their health condition.
- Set SMART health & well-being goals to improve quality of life.
- Better navigate the health care system to access services.
- Prevent or reduce complications arising from chronic health conditions.
We offer access to Allied Health Services; 6 – 10 week exercise & pain management programs; health literacy support and remote monitoring of health vital signs.
Who this service is for
People living in the Derwent Valley, Central Highlands or Southern Midlands, who have any of these chronic health conditions:
- Circulatory and heart disease (cardiovascular)
- Chronic lung disease (including COPD & asthma)
- Musculoskeletal disorders (including osteoporosis; osteoarthritis)
- Diabetes (including gestational; pre-diabetes condition)
There is no age restriction.
This program is supported by funding from Primary Health Program (Tasmania PHN), through the Australian Government’s Primary Health Networks Program.
Cost
Free.
A gold coin donation may be requested for some exercise programs.
